Penn Center for Community Health Workers seeks Project Manager

About the organization: The mission of the Center is to improve health in high-risk populations through the effective use of Community Health Workers (CHWs). CHWs, hired from within local communities, provide social support, advocacy and navigation to low-income, high-risk patients. Our Center has developed IMPaCT, a CHW program which has been scientifically proven to improve patients’ access to care, engagement with healthcare, and hospital readmissions. We are a nationally-recognized CHW leader and have been featured in the New York Times, National Public Radio, and Forbes magazine. Our Center is part of the University of Pennsylvania Health System.

About the position: In response to our success within Penn Medicine and training requests from organizations across the country, the Center is growing. As a result, we are inviting applications from energetic and passionate individuals who want to supervise a team of Community Health Workers, help us continue to innovate and refine IMPaCT, and influence new models of healthcare for marginalized communities in Philadelphia and across the country.
