Job Overview:
The Coach works with the Code for Success team to provide wrap-around services to youth enrolled in job training and readiness programs.
This person maintains frequent contact with participants through in-person meetings, texting, phone calls, and video conferencing. This person will be responsible for intake assessments, identifying and setting goals with participants, making appropriate referrals for supportive services, and completing periodic progress assessments, including closing assessment, to document participant progress. This person will also make warm transfers for services and/or accompany participants for health and social services appointments, as appropriate.
